How to complete the team’s roster form

Follow the instructions below on how to complete the blank roster form for the season.  Note: This information may not be complete; please refer to the registration platform for more information.

Be sure to provide complete information and return the completed roster to the League as an Excel document (not a Google document or in the body of an email, etc.) no later than the Roster deadline (as listed on the website).  Failure to do so may result in forfeiture of the team’s $200.00 deposit.  (Note:  If a Captain needs the spreadsheet converted to an earlier version of Excel, please contact the League).

    • Player # – each team is charged for a minimum number of Players (15 for outdoor, 12 for indoor), but may choose to have fewer Players on their team’s roster.  There is no maximum to the number of Players who may be on a roster.
    • Jersey # – these should be listed in ascending order.  This will enable the Captain to identify if more than one Player has been assigned the same number.
      • Jersey numbers must be selected from 1-99 only.
      • It is recommended that each Captain assign new players to their team a number that has not previously been assigned.  See Shirt Policy for details.
    • New – indicate whether the Player is new to your team (not to the League).  If the Player is new to your team, please indicate this with a “Y” and they will receive a jersey.
      • If a Player is new to your team, but they have a jersey of your team’s color and the number does not conflict with one of your other Players’ numbers, please put “N”, and that player will not be considered a new player (and will get the returning player price).
    • Player Name – provide the Player’s full first and last name.
      • Please indicate with a “(G)” next to the Player’s name if that Player is one who can regularly play goalie for your team.
    • Date of Birth – provide the Player’s date of birth.
      • As noted in the rules, all Players must be at least 21 years of age as of the beginning of the session in which s/he plays in a League game.
    • Phone Number – provide the Player’s preferred phone number.
    • Sex (M/F) – indicate whether the Player is male or female.
    • Jersey Size – provide the Player’s preferred jersey size.  (Note:  all jerseys are men’s sizes; there are no different jersey sizes for women).  See Shirt Policy for details.
    • Email Address – provide a valid email address that the Player checks regularly.
    • Already Signed Waiver – if a Player has played in the League before, whether or not on your team, they should have signed a League waiver.  If the Player has signed a waiver, they do not need to sign another one; kindly put “X” in this column.  If the Player has not signed a waiver, please leave this blank.
      • If you or the Player is not sure, that Player should sign a waiver to err on the side of caution.
      • The League will indicate which, if any, players on the roster need to sign a waiver in the email containing the team invoice.
    • Color Preferences – the League’s priority is to provide shirt colors to teams such that two teams will not have the same color uniforms.  In light of this, the League will make a concerted effort to provide each team with its desired color of uniform, but makes no guarantee to do so.
      • New teams – Below the roster section, you should rank your top five (5) color preferences.  Click here to visit the website for the shirts.
      • Returning teams – unless asked by the League to change your team’s shirt color, your team will maintain its prior color.  Teams may inquire about changing shirt colors, but such a change would need to be approved and would require players to cover the cost of replacement shirts.  See Shirt Policy for details.

Note to Captains:

  • Please make sure that non of the Players listed on your roster have registered separately as a Free Agent as those Players may be ineligible for your team.  If a Player has already registered as a Free Agent, the League must approve that Player’s removal from the Free Agent pool and subsequent placement on your team.  See Registration Policy for details.
  • If you need to add any Free Agent players to complete your team, kindly inform the League as soon as possible and the League will do its best to assist with Free Agent assignment.  See Registration Policy for details.
  • Additional notes:
    • If you have the majority of the roster information complete and you are missing a few phone numbers or DOB’s, that’s okay, but you are responsible to provide that information as soon as possible.
    • If jersey sizes and numbers are not provided by the deadline date, your players risk a delay in when their jerseys will be ready.
