Documentation Policy

Required Information

  • All information (including documentation) required to be submitted to the League should be done in a timely manner and in accordance with the deadlines set by the League:
    • Roster information – by the roster deadline (see Roster Policy)
    • Waivers – must be signed by players before participating in a game and submitted to the League in a timely manner (see below)
    • Non-Roster Player form – by kickoff of each game, given to the ref; email with info for all players listed on the Non-Roster Player form – by Tuesday night at midnight
  • It is the responsibility of Captains to ensure that all required information (including forms) is provided in accordance with the League rules.  It is NOT the responsibility of the League to police the teams to ensure compliance, nor is it the responsibility of the League or the Commissioner to provide forms to teams or players beyond making them available on the League’s website.
  • Failure of a team to submit required information will result in the team, team’s Captain, or players being subject to disciplinary action at the discretion of the League.

Waiver Form

  • The waiver form is available at the top of the main rules page.
  • ALL waivers for all roster players not on file with the League must be given to the League by the first week of the season, regardless of whether that player will be at the first game.
    • Failure to have all waivers for roster players submitted by the first game of the season may result in that team’s suspension of the privilege of non-roster players until all such waivers are submitted.
    • NOTE:  While it is the responsibility of the team’s Captain and the individual players for ensuring that all necessary waiver forms are signed, the Commissioner does make a concerted effort to assist in the process by communicating with the team’s Captain those players (if known) who need to sign a waiver.
  • Each Player must sign (or have signed) a League waiver form before participating in any League game.
    • If a Player has signed a League waiver in the past, another one need not be signed.
    • At this time, e-signatures are not being accepted, except as a placeholder until the player shows up to their first game and can sign in person.  In all cases, hard copies of signed waivers are preferred over pictures, etc.; however, pictures will suffice as a temporary placeholder that will suffice for the Tuesday deadline.
  • Signed waiver forms shall be given by that Player or by the Captain to the League commissioner (or another appointed League official, if any) prior to that Player taking the field, to the extent practicable.
    • If the commissioner (or any other League official, if any) is not available at the field, the Captain should either (1) deliver the signed waiver to the commissioner, or (2) send a scan of the signed waiver to the League via email no later than Tuesday following the game.  The Captain should hold the original to give to the commissioner at the earliest reasonable opportunity.
  • Waivers should NOT be given to referees.
    • My mailbox is literally within walking distance of Patterson Park; it should not be difficult to drop off a waiver in my mailbox after a game.
  • This is a serious liability issue; if someone doesn’t get hurt during a game and they sign a waiver afterward, it’s fortunate, but if the alternative happens and they get hurt without signing a waiver, bye bye League.  If you’re ever not sure if someone needs to sign a waiver, err on the side of caution and have them sign it!
    • Note: if a player plays with a team in the game before you, there’s no guarantee that the Captain of that team had them sign a waiver.

[New for 2019] Waiver Form – Not Signed / Not Received Timely

  • Waiver not signed by Player prior to playing.  Failure of a Player to sign a waiver before participating in a game will result in the following:
    • If the Player scores a goal prior to signing a waiver, that goal will not count; and
    • The Player and the team for which s/he played may be subject to disciplinary action at the discretion of the League.
  • Waiver signed, but not submitted timely to the League.  If a waiver is not received by the League…
    • In a timely manner (see above) – any goals scored by that Player in subsequent games will not count for the period that the waiver is outstanding.  Goals will count for that Player starting from the time the waiver has been received by the League.
    • Prior to the team’s last regular season game – the team may be prevented from playing a playoff game.
    • Prior to the team’s final game of the season (including playoffs) – the team may forfeit the team’s Forfeit Fine held on account with the League.  See the Forfeit Policy.
    • The Player and/or the team for which s/he played may be subject to disciplinary action at the discretion of the League.

Non-Roster Player Form

In order for a Player who is not on a team’s roster to be eligible to play in that team’s regular season game, a team must abide by the requirements set forth in the Non-Roster Player Policy.

Failure of a team to follow these requirements will result in the following:

  • [New for 2019] If a Player is not listed on the non-roster form and that Player scores a goal, the goal will be disallowed after the conclusion of the game.
  • If these procedures are not followed for a team’s final regular season game, the team may be subject to a forfeit (including loss of Forfeit Fine) for that game.
  • The team may be subject to disciplinary action at the discretion of the League.
